HIPPA and your records

Your health information, whether in paper or electronic form, is protected under the Health Information Portability and Accountability Act (HIPPA, 1996). Because counseling records are considered health information I have taken every effort to protect your privacy and confidentiality. Effective January 1, 2013, my office procedures are paperless so as to use the most secure, encrypted, and confidential means available to mental health professionals today to protect your privacy. The service I use to manage and store your information is HIPPA compliant meeting stringent federal regulations for storage of personal electronic medical records.

Adults/Couples Registration

After registering as a new client, you will receive an email with password information and a link to your own personal portal page. Click on “forms” and immediately read and electronically sign the Informed Consent and Disclosure before completing the Family/Behavior/Mood questionnaire. Each person of a couple must complete his/her own registration process.

Children and Teens Registration

A parent will submit registration using the child’s name and will receive an email with password information and a link to a personal portal page. Click on “forms” and immediately read and electronically sign the Informed Consent and Disclosure. Older teenagers may complete the Family/Behavior/Mood questionnaire on their own or you can do it together. For younger children the parent should complete only the questions appropriate to their child’s situation or developmental stage.

Secure email

After registering online, you can send email to and receive email from Patty from your personal portal.  All email is encrypted to HIPPA regulations and will never be accessible to anyone else.

Texting

Texting is not a secure way to communicate.  If you text me, you cannot be sure I will receive it promptly.  Please call and leave a voicemail for appointment related matters, send a secure email from your portal, or use the calendar/scheduler on your portal.
